What is evidence of identity?
When applying for, or renewing, a Queensland driver licence you must show an existing Queensland driver licence (current or expired less than two years). If you can't show an existing Queensland driver licence, you must provide sufficient evidence of identity.
You must also provide sufficient evidence of identity when you are applying for a replacement Queensland driver licence because it has been lost, stolen, destroyed or defaced, or you have changed your name and you want your new name shown on your driver licence.
You will need to show at least three 'evidence of identity' documents, including:
- one category A document
- one category B document
- one from either category A or B.
and
At least one document must show your signature. All documents must be current unless otherwise stated. Only original documents can be accepted.
If your current Queensland residential address is not stated on any of these documents, you must show one document with evidence of your Queensland residential address.
The identification requirements for the issue of a Queensland licence meet the national standards, which ensure that the security of the driver licence system is maintained and the potential for fraud is minimised.
You must give the correct information when you apply for a licence. If you do not, your licence will be cancelled. If you change your name or address, you must notify the Department of Transport and Main Roads within 14 days of the change.

If you are a new customer to the department you will also need to complete a New Customer Application form (F3503).
Category A documents
Category A documents establish the legal existence of your name and date of birth. These documents may include your:
- Australian birth certificate — full (commemorative certificates, extracts, acknowledgment of birth, photocopies or certified copies of original documents are not acceptable)
- Australian or foreign passport (current or expired less than two years)
- Australian citizenship or naturalisation certificate
- Australian photo driver licence (current or expired less than two years)
- Department of Immigration and Citizenship travel document (valid up to five years), for example, resident visa
- Department of Immigration and Citizenship certificate of evidence of resident status
- Australian Defence Force photo identity card
- Queensland or federal police officer photo identity card
- Queensland 18+ Card (issued after 1 January 1992)
- Department of Transport and Main Roads driver authorisation (current or expired less than two years)
- Department of Transport and Main Roads accreditation, for example, rider or driver trainer accreditation, tow truck accreditation (current or expired less than two years).
Category B documents
Category B documents establish the use of your identity in the community. Only Category B documents showing the full first name are acceptable. These documents include your:
- Medicare card (may be used by any person listed on the card)
- Department of Veterans' Affairs/Centrelink pensioner concession card (including health care cards)
- financial institution debit or credit card with your signature and embossed name (must show full first name)
- Australian security guard/crowd controller licence (with photo)
- Australian firearm licence (with photo)
- educational institution student identity document (must include photo and/or signature).
Evidence of Queensland residential address
If your current Queensland residential address is not shown on one of the category A or B documents, you must show an 'evidence of Queensland residential address' document. These documents include your:
- Australian Taxation Office assessment (last or current years)
- Australian Taxation Office tax file number confirmation (valid for two years from date of issue)
- contract of purchase, current lease or rental document, mortgage or land ownership certificate
- Queensland land tax valuation notice
- Queensland local government rates notice
- Queensland vehicle registration certificate
- Queensland Driver Licence or Vehicle Registration Renewal Notice for coming period
- electricity, gas or telephone account
- bank statement issued from the same financial institution as debit/credit card supplied (issued within the last six months).
If you are genuinely unable to show one of the above documents, you may:
- complete the Queensland Residency Declaration (F4208)
- show a statement from your educational institution's administrator (if you are a student)
- show a statement from your employer.
Change of name requirements
If you have changed your name or the details of your name are different on the evidence of identity documents to be shown, you must show an official 'evidence of change of name document' from a Registrar of an Australian Births, Deaths and Marriages.
These documents include:
- a change of name certificate issued by Registrar of an Australian Births, Deaths and Marriages
- a marriage certificate that is registered and issued by Registrar of an Australian Births, Deaths and Marriages (ceremonial certificates are not acceptable)
- an amended birth certificate (with notations) issued by Registrar of an Australian Births, Deaths and Marriages.
Overseas marriage certificates
An official overseas marriage certificate is acceptable if presented with either:
- a category A document in your married name
- two category B documents in your married name.
or
The overseas marriage certificate must show an official crest and be appropriately registered.
Divorce documents
Divorce papers 'decrees nisi' or 'decrees absolute' may be acceptable providing they show the name being reverted to. Deed polls issued before 1 February 2004 are also acceptable.
Statutory declarations
If you have previously changed your name by declaration or statutory declaration, please contact the department on 13 23 80b to discuss the required documentation. Only original documents can be accepted.
What if I am unable to show any evidence of identity?
If you are unable to show any of the required evidence of identity documents, phone the department on 13 23 80b or visit a Department of Transport and Main Roads customer service centre, authorised Queensland Government Agency Program Office or licence issuing police station.
