Identification requirements

Browse the links below to jump to the information you need.

What is evidence of identity?

When applying for, or renewing, a Queensland driver licence you must show an existing Queensland driver licence (current or expired for less than two years). If you can't show an existing Queensland driver licence, you must provide sufficient evidence of identity.

You must also provide sufficient evidence of identity when you are applying for a replacement Queensland driver licence because it has been lost, stolen, destroyed or defaced, or you have changed your name and you want your new name shown on your driver licence.

You will need to show at least three 'evidence of identity' documents, including:

At least one document must show your signature. All documents must be current unless otherwise stated. Only original documents can be accepted.

If your current Queensland residential address is not stated on any of these documents, you must show one document with evidence of your Queensland residential address.

The identification requirements for the issue of a Queensland licence meet the national standards, which ensure that the security of the driver licence system is maintained and the potential for fraud is minimised.

You must give the correct information when you apply for a licence. If you do not, your licence will be cancelled. If you change your name or address, you must notify the Department of Transport and Main Roads within 14 days of the change.

To find out what you have to do to comply with the new evidence of identity requirements for driver licensing, see the information below or download the Evidence of Identity Information Sheet (form F4362).

If you are a new customer to the department you will also need to complete a New Customer Application (form F3503).
