
Information for health professionals
As a health professional, you may be required to provide a medical opinion on a patient's fitness to drive.In a recent review of medical conditions reporting, Queensland Transport has revised the Medical Certificate for Motor Vehicle Driver form (F3712), and the Private and Commercial Vehicle Driver's Health Assessment form (F3195).
The following medical conditions reporting information kit provides information on the changes to these forms. Also included in the kit are a number of guides that are designed to aid health professionals and patients to complete the revised forms.
