Replacing a licence

If your existing licence has been lost, stolen, destroyed or defaced, a Replacement Licence Application/Address or Renewal Label Application form (F3006) must be completed. A fee is charged.

You must also apply for a replacement licence if a Magistrates Court has granted a special hardship order.

If you have changed your name and you want your new name shown on your driver licence a Change of Customer Details (Individuals) form (F4214) must be completed. A replacement licence will be issued to you in your new name at no charge.

You will need to prove your identity when you are applying for a replacement licence.

Where you can replace your licence

You can apply for the replacement licence at a Queensland Transport customer service centre. If you live in a rural area, you can apply at a Queensland Government Agency Program (QGAP) office or licence-issuing police station.
