Replacing a licence

If your existing licence has been lost, stolen, destroyed or defaced, you must complete a Replacement Licence Application/Address or Renewal Label Application form (F3006) and pay a fee.

You must also apply for a replacement licence if a Magistrates Court has granted you a special hardship order.

If you have changed your name and you want your new name shown on your driver licence, you must complete a Change of Customer Details (Individuals) form (F4214). A replacement licence will be issued to you in your new name at no charge.

You will need to prove your identity when you are applying for a replacement licence.

You can apply for the replacement licence at a Department of Transport and Main Roads customer service centre. If you live in a rural area, you can apply at a Queensland Government Agency Program office or licence-issuing police station.
